The appointment SMS program will begin only after required messaging approvals and valid consent.

Text messaging is optional. Consent is not a condition of treatment and is separate from email communications and treatment consent. Declining SMS does not affect treatment.

Message frequency varies. Message and data rates may apply.

Reply STOP at any time to opt out. STOP suppresses appointment texts; a confirmation of opting out may be sent.

Checking a box cannot automatically reverse an opt-out. Re-enrollment must follow a documented valid new opt-in and the applicable messaging provider process, not an automatic reset.
